Treatment Planning
By using a team approach, staff working directly with that consumer, develop an individualized treatment plan specific to that individual. This treatment plan identifies that consumer’s strengths and needs. From the identified needs, a person centered plan is developed that includes measurable and time specific goals, objectives and interventions. Interventions are developed with consideration of the the consumer’s strengths and consumer identified areas of need.
Treatment plans are reviewed a minimum every 3 months and completely reviewed annually. In the event of any major life change or at the time of an identified need, amendments are made to ensure that the consumer is receiving the most appropriate care and treatment for them at that time.
